LONG-TERM DISABILITY
TMCF offers full-time team members a non-contributory long-term disability (LTD) base plan. This non-contributory base plan currently provides for monthly LTD benefits of 60% of basic monthly earnings to a maximum benefit of $6,000 per month, less any other offsets. Eligible team members are automatically enrolled as of the first day of the calendar month on or following their date of hire. Long-term disability coverage terminates on the last day of employment.
SHORT-TERM DISABILITY BENEFITS
TMCF’s short-term disability (STD) plan is a benefit that provides partial pay for team members who are unable to work due to a covered non-work-related illness, injury or disability, after an absence of more than seven consecutive calendar days. STD benefits pay up to 60% of covered weekly earnings (up to $2,308 per week). Coverage begins on the 15th day of disability. A team member seeking benefits must file a leave-of-absence form with Insperity, which will begin the paperwork process. A short-term disability leave must be certified by a physician’s or licensed healthcare professional’s statement identifying the nature of the disability and stating or estimating the date when the team member will be able to return to work. If the team member cannot return on that date, another statement from a physician or licensed healthcare professional, with a new return date will be required. Team members will not be able to return to work without submitting a note, to the human resources department, from a physician or licensed healthcare professional authorizing the team member’s return.
TMCF reserves the right to confirm the necessity for disability leave by requiring the team member to receive a second or third opinion by a doctor of TMCF’s choice. TMCF will assume any costs for additional examinations that are not paid by the insurance carrier. Any FMLA leave, to which a team member may be entitled, runs concurrently with time off under TMCF’s short-term disability policy.
WORKERS’ COMPENSATION
TMCF provides workers’ compensation benefits in accordance with and subject to applicable law. Team members who sustain work–related injuries or illnesses must inform their supervisor immediately. No matter how minor an on-the-job injury may appear, it is important that the injury be reported as soon as practicable, typically no later than 24 hours after the injury or illness occurred. Unless otherwise required by applicable law, neither TMCF nor any workers’ compensation carrier will be liable for the payment of workers’ compensation benefits for injuries or illnesses that occur during a team member’s voluntary participation in any off-duty recreational, social or athletic activity, whether or not such activity is sponsored by TMCF. Team members are required to sign a waiver of the right to sue. For more information, contact the human resources department.
